Affected creatures that are lying or sitting will stand when affected by the spell. Creatures that are bleeding from wounds, or acting in some manner, will cease what they're doing and stand, watching the patterns passively as the spell continues.
If attacked hand-to-hand, however, affected creatures are entitled to a surprise check; if they're not surprised, the hypnotic pattern spell is broken and these creatures are also allowed to roll initiative to defend themselves. A missile attack can be made against the creature without allowing surprise or initiative, but once damage is done the spell is again broken.
