Hypnotic Pattern creates a panoply of weaving, turning patterns in the air, located within the spell's range. These patterns are so compelling that creatures designated by the caster must make a saving throw, or else stand gazing, hypnotised, by the effect. During this time, they will take no action for as long as the caster maintains concentration.

If attacked hand-to-hand, however, affected creatures are entitled to a surprise check; if they're not surprised, the hypnotic pattern spell is broken and these creatures are also allowed to roll initiative to defend themselves. A missile attack can be made against the creature without allowing surprise or initiative, but once damage is done the spell is again broken.
